Customer acknowledges and understands that U4VOIP’s liability for any Service outage and/or inability to dial Emergency Calling Service from your line or to access emergency service personnel, as set forth in this document. Customer agrees to defend, indemnify, and hold harmless U4VOIP, its officers, directors, employees, affiliates and agents and any other service provider who furnishes services to Customer in connection with this Agreement or the Service, from any and all claims, losses, damages, fines, penalties, costs and expenses (including, without limitation, reasonable attorneys fees) by, or on behalf of, Customer or any third party or user of Customer’s Service relating to the absence, failure or outage of the Service including Emergency Calling Service dialing and/or inability of Customer or any third person or party or user of Customer’s Service to be able to dial 911 or to access emergency service personnel. This paragraph shall survive termination of this Agreement. |
LNP/LOA Porting Agreement |
I certify that I have read and understand this Letter of Agency. I further certify that I am at least eighteen years of age, and that I am legally authorized to change the telephone service providers for services to the telephone numbers listed above. I understand and agree that I may be subject to a one-time charge per line for each change in provider. (I may consult with the carrier as to whether the charge will apply.) If I later wish to return to my current provider, I may be required to pay a reconnection charge to that company. I also understand that my new service provider may have different calling areas, rates and charges than my current provider, and that by signing below I indicate that I understand those differences, (if any) and am willing to be billed accordingly. I agree to the terms of the current service agreement and it is my responsibility to keep myself updated with the most recent terms and conditions as they are subject to change periodically. |
Provider's Service Agreement |
I understand that U4VOIP will prorate my first month’s usage and bill all U4VOIP services one month in advance. U4VOIP will produce all bills electronically. I am responsible for checking my email and retrieving invoices. It is my responsibility to notify U4 if I do not receive these invoices. Payments will be made electronically with the credit card number I provide to U4 Customer service and my credit card will be charged automatically each month thereafter. I understand that billing starts when this contract is agreed. There is a $5.00 late fee for payments received after the due date. There is a minimum one month of service required. In the event that you default on your monthly payments, your service will be suspended until the current balance is paid. If payment has not been received within two weeks after suspension, my account will be terminated. Some requested changes to your account may incur a $20 administrative fee. I understand that I am billed for service regardless of usage until my service is terminated. I understand that any balance not paid will be turned over to collections and I will be responsible for the outstanding balance as well as collection fees, attorney fees, court costs and legal interest. U4VOIP service is subject to a $30 activation fee. All VOIP circuits are subject to a $1.55 FCC Regulatory Fee. |
